| Patient Preparation | Provide patient with instruction sheet and labeled specimen container(s). If M&C is requested, a single specimen in a brown top faeces container is required. If O,C,P requested, collections on two separate days are required (provide a brown top and a blue top container for each collection). | ||
| Collection Instructions | Patient instructions Note: The fluid in the blue top container is hazardous. Keep out of reach of children. Specimens can be collected on any day Monday to Friday and must be delivered to collection centre on the same day (refer to reverse of request slip for collection centre hours) 1. Please write your full name, date of birth, date and time of collection on the jars provided. 2. It is important that the specimen is not contaminated with urine or water from the toilet bowl. Use an old ice-cream container or similar receptacle to catch the specimen. 3. Open the brown top jar. Using the scoop built into the lid, place a scoop of faeces into the jar. Close lid tightly. 4. Open the blue top jar with care as it contains liquid chemical preservative. Using the scoop built into the lid, place a scoop of faeces into the liquid. Close the lid tightly then shake gently to ensure thorough mixing. 5. Place specimen(s) and paperwork in plastic bag provided and deliver to your local collection centre on the day of collection. | ||
